Chronic Kidney Disease (CKD) ProgramWelcomeWelcome to our program. Your kidney doctor has recommended you become involved in The Kidney and Hypertension Center’s chronic kidney disease (CKD) program. The CKD program is a team approach. You visit with a nurse practitioner in between visits with your kidney doctor. Nurse practitioners are registered nurses with advanced education. They are qualified to adjust medications and write prescriptions. Our nurse practitioners have experience in caring for patients with kidney disease and are able to address many of the common health conditions associated with this chronic condition. Recent information shows that there are nearly 20 million people in the United States with early stages CKD. You can have a lot of kidney damage and not even feel ill. That is why kidney disease if often called a silent disease. Therefore, it is very important to monitor your kidney function through regular follow-up including laboratory testing. The Kidney and Hypertension Center’s CKD program was developed to address the growing needs of our patients. Our mission is to work with you to optimize your care for the purpose of slowing progression of this chronic health problem. We offer the following services:
