Physician Consultations

We are pleased to assist you in the care of your patient's nephrology and hypertension issues. We are available throughout the Tristate region, including Southwestern Ohio, Northern Kentucky and Southeastern Indiana. .

As described elsewhere, we offer your patients more than 300 years of collective experience and comprehensive nephrology care. We see patients in both the hospital and outpatient settings and are happy to treat the entire spectrum of renal diseases as well as uncontrolled hypertension.

The majority of our patients have some form of chronic kidney disease, and our goal is to preserve as much renal function as possible; if they progress to needing dialysis, we will work with your patient to find the appropriate dialysis modality, including incenter hemodialysis, peritoneal dialysis, or home hemodialysis; and if appropriate, we will also assist your patient in his/her pre-transplant and post-transplant needs.

The form should be attached to an e-mail message and returned to referral@khc.cc. This informs us of the consultation as well as the importance to watch for the patient's medical information which should be faxed or mailed to one of the following addresses:

2)We will contact the patient and schedule him/her for the earliest possible appointment. If there is a specific urgency to the consultation, please specify it on the consultation request, so we may work with the patient to find a suitable date and time.
3)After we have met with the patient, you will receive a written or verbal communication from one of our physicians regarding our assessment and diagnostic or treatment plan. You will also receive a faxed summary of each subsequent visit.
